I understand that this service will only be rendered if a prescription for home care is obtained from your veterinarian by SitnHeal. I agree to provide all necessary supplies. A refund will be issued if service cannot be rendered (enter your name below)

Take a small amount of blood from the patient to check blood sugar value. Specialized treatment for puppies, diabetic patients, or when medically indicated. All supplies needed for glucose check must be provided by the owner (needles/syringes, glucometer) or the service fee for the blood glucose check will be refunded

*Service can only be provided with prescription from your current veterinarian, SitnHeal partners with veterinary clinics to provide Certified Veterinary Technicians for home care under the supervision of your veterinarian. SitnHeal will contact your veterinarian for a home care specific prescription, and this service is dependent on these permissions.
